Qualitative Analysis of Peer Coaches’ Experiences with Counseling African Americans About Reducing Heart Disease Risk.

BACKGROUND: Despite mounting evidence that peer coaches can make significant contributions to patient health, little is known about factors that must be addressed to engage and retain them in their role. OBJECTIVE: To identify motivators and barriers to serving as a peer coach. DESIGN: Open ended semi-structured interviews. PARTICIPANTS AND SETTING: In a randomized trial of peer support, patients with well controlled hypertension and good interpersonal skills were recruited and trained to serve as peer coaches for African-American patients from the same practices who had poorly controlled hypertension. Peer coaches spoke by telephone at least three times with their same sex patient-clients on alternate months during the 6-month intervention and counseled about medication adherence as well as other healthy lifestyles. KEY RESULTS: Of 15 trained peer coaches, ten were contacted and agreed to participate in the qualitative interview. Peer coaches had a mean age of 66 years, 50% were women, and 80% were African-American. Themes regarding favorable aspects of the peer coach experience included: meaning and satisfaction derived from contributing to community health and the personal emotional and physical benefits derived from serving as a peer coach. Negative aspects centered on: challenges in establishing the initial telephone contact and wanting more information about their patient-clients’ personal health conditions and status. Peer coaches endorsed gender matching but were less clear about race-matching. CONCLUSIONS: Programs that utilize peer support to enhance positive health behaviors should recognize that a spirit of volunteerism motivates many successful peer coaches. Program planners should acknowledge the special characteristics required of successful peer coaches when selecting, motivating and training individuals for this role.

ABSTRACT BACKGROUND:Comorbid ischemic heart disease (IHD) is a common and important cause of morbidity and mortality in COPD patients. The impact of IHD on COPD in terms of health status, exercise capacity and symptoms is not well understood. METHODS:We analyzed stable state data of 386 patients from the London COPD Cohort between 1995 and 2009 and prospectively collected exacerbation data in those who had completed symptom diaries for ≥one year. RESULTS:Patients with IHD (n=64,16.6%) had significantly worse health status as measured by SGRQ (56.9±18.5 vs 49.1±19.0, p=0.003) and a larger proportion of this group reported more severe breathlessness in the stable state with an MRC dyspnea score of ≥four (50.9% vs 35.1%, p=0.029). In subsets of the sample, stable COPD patients with IHD had a higher median (IQR) serum NT-proBNP concentration than those without IHD (38 (15,107) vs 12 (6,21) pg/ml, p=0.004), and a lower exercise capacity (6MWD 225±89m vs 317±85m, p=0.002). COPD exacerbations were not more frequent in those with IHD (1.95 (1.20,3.12) vs 1.86 per year (0.75,3.96), p=0.294), but the median symptom recovery time was five days longer (17.0 (9.8,24.2) vs 12.0 (8.0,18.0), p=0.009), resulting in significantly more days per year reporting exacerbation symptoms (35.4 (13.4,60.7) vs 22.2 (5.7,42.6), p=0.028). These findings were replicated in multivariate analyses allowing for age, gender and FEV(1,) and exacerbation frequency where applicable. CONCLUSIONS:Comorbid IHD is associated with worse health status, lower exercise capacity and more dyspnea in stable COPD patients and longer exacerbations, but not an increased exacerbation frequency.

if there was a disease that affected only boys, would there be such an outcry for a vaccine? or would it be ignored because “only men” suffered, so who cares?
it seems to reflect current societal standards that women get everything, and all men are to be punishewd
So, not only are baby boys expected to be circumcised to reduce HPV for the sake of women, but girls get a free vaccine that boys dont even get.
think about it…BABY boys are punished for women forgetting to use condoms. we have to have 20,000 nerve endings ripped off at birth causing lifelong damage in case ADULT women are irresponsible with sexual health decision.
also, why are only girls getting the vaccine considering another medical fact: women live longer than men due to biological differences. womenare less likely to get cancer, heart disease, die early, etc.
since women have a longer life expectancy, is it really fair to give focus EXCLUSIVELY to women? shouldnt men, who die much younger to begin with, get something to?
